I am in complete understanding that my child is participating in the above mentioned activities sponsored by the YWCA Central Massachusetts. I fully understand and choose not to, and will not hold the YWCA Central Massachusetts, any of its agents, assigns, employees, or volunteers liable for any accident, injuries, or any other unforeseen harms incurred at any time while participating in this activity. I authorize YWCA staff to find adequate and reasonable medical treatment at my expense. If the need arises: this waiver will serve as a medical release form, thus authorizing YWCA staff permission to act on my behalf until such a time that I can be contacted.

We does not offer regular transportation to and from normally scheduled programming held at the YWCA Central MA. In case of extreme emergency (such as parent hospitalization, death in the family, etc.), transportation may be available, subject to approval by staff. Parents/Guardians may arrange for possible emergency transportation by contacting staff directly.

I acknowledge that the YWCA staff is not liable for any injury to person or possession during field trips, activities, or in cases of emergency. I give permission for YWCA staff to transport my child in case of extreme emergency.
